Community Living Center
Skilled rehabilitation and subacute levels of medical care are provided to eligible veterans in the Community Living Center (formerly known as Long Term Care). Care is aimed at meeting the needs of those who no longer require acute hospitalization. The focus is on restoring or maintaining the highest degree of functional independence possible. Other services include providing respite to the caregiver to support the veteran who live in the community and compassionate care and support to veterans and families through the dying process.
Each veteran receives care according to a "Plan of Care". This individualized plan is developed by the veteran along with a team of professionals. Team members include physicians, nurses, dieticians, social workers, physical therapists, speech therapists, and pharmacists.
Residents have input into decisions that impact their living environment through a Resident Council. Daily, they can participate in a number of recreational and leisure activities under the direction of a Recreational Therapist.
Visitors are welcome and encouraged. Although, the best time is from noon until 8pm, residents may have visitors as long as the privacy and rights of other residents are not affected.
